When your teen is struggling with anxiety, finding the right therapy can feel overwhelming, but it is essential. Two of the more commonly recommended therapies are Cognitive Behavioral Therapy (CBT) and Dialectical Behavioral Therapy (DBT). Both are evidence-based and effective, but work in different ways to meet differing needs.

Understanding the key differences between these two approaches can help you and your teen make an informed decision about which path to take. Let’s explore these differences further and see how each one addresses anxiety in teens.

What is CBT?

girl-sitting-on-grass-in-summer-and-smilingCBT focuses on the connection between thoughts, feelings, and behaviors. It centers on the idea that our thoughts dictate our emotions and behaviors. When we start to feel anxious, these thoughts create a vicious cycle that ends up fueling anxiety.

In CBT, your teen will learn how to:

  • Identify anxious thought patterns
  • Challenge unrealistic or unhelpful thoughts
  • Replace negative thinking with more balanced perspectives
  • Practice new behaviors that reduce anxiety

CBT is structured and works well with teens who need a more direct approach. There are often homework assignments between sessions, like tracking anxious thoughts or practicing exposure to feared situations. It can hone in on specific goals and provide the tools needed to reach them.

What is DBT?

DBT combines certain CBT principles with additional mindfulness practices. It focuses on emotional regulation and interpersonal skills. Originally developed for individuals with intense emotional experiences, it has found to be effective in treating anxiety at all levels.

DBT teaches four main skill sets:

  • Mindfulness: staying present in the moment
  • Distress tolerance: managing difficult emotions without making things worse
  • Emotional regulation: understanding and managing intense feelings
  • Interpersonal effectiveness: communicating needs and maintaining relationships

For anxious teens, DBT provides the tools necessary for tolerating uncomfortable feelings rather than trying to ignore them completely. It acknowledges that difficult emotions are a part of life and equips them with the ability to accept certain feelings and adapt accordingly.

What Approach is Right for Your Teen?

CBT may be the better fit if your teen is struggling with anxiety driven by fear or specific thought patterns. For example, your teen may have intense worries about academic performance or social situations. Perhaps they are triggered by a specific phobia. CBT can use that information and help restructure their thought patterns to be able to navigate these challenges.

DBT might be more helpful if your teen is experiencing intense mood swings along with anxiety,  thoughts of self-harm, impulsive behaviors, or difficulty managing overwhelming feelings. The emphasis on emotional regulation and distress tolerance can provide a healthy foundation for addressing anxiety in a broader emotional context.

Some teens may find more benefit in a mixture of both approaches. Many therapists integrate techniques from both CBT and DBT, making the treatment plan more personalized to fit your teen’s unique needs. There is no cookie-cutter, one-size-fits-all method in therapy.

The Somatic Connection

We recognize that anxiety lives in the body as much as in the mind. Whether you and your teen are interested in pursuing CBT, DBT, or a combination of the two, incorporating somatic awareness can help boost the healing process. Increasing awareness of physical sensations, using breathwork, and releasing tension held in various areas of the body will enhance both therapeutic approaches.

Your teen’s anxiety doesn’t have to be a defining factor during these pivotal years. With the right support, they can develop tools to help navigate any anxious feelings and restore a healthy sense of peace and confidence.
